5th grade Behavior Management Plan

Level 1 Class Level Interventions
- Determined by individual teachers in the classroom
  • o May include verbal warnings, change of seat, private conversations with the teacher, proximity intervention, etc.

Level 2 Attitude Adjustment/ Behavior Incident Report
- Grade wide plan – when the behavior is more severe, repeated, or otherwise needing a graduated response
- Attitude Adjustment: (thought on this name?) This is an intervention in which students can reflect on their behavior. There will be a teacher assigned as the Attitude Adjustment Officer each week (or possibly it will be a duty?). It will be this teacher’s responsibility to discuss with the student what she/he will do. Where possible and appropriate the student should decide the task themselves.
  • o The guiding principles are responsibility, self-determination, and respect
- Behavior Incident Report: This is a standardized form that is sent to parents informing them of the student behavior in question.
- It is suggested that if one of these interventions is used then they both are, however, it will be at the teacher’s discretion if he/she decides only one is necessary in a given case.

Level 3 Office Referral/ Parent Meeting
- Grade wide plan – when the behavior is more severe, repeated, or otherwise needing a graduated response
- One or both of these interventions may be used, however it will be at the teacher and/or SA director’s discretion if it is one or both in a given case.
